Project Title: Community Health Workers Using Patients Stories to Support Hypertension Management

Project Description:
The overall goal of this project is to develop an intervention that integrates storytelling with a traditional Community Health Worker (CHW) intervention. CHWs, sharing a common cultural/historical background with members of their communities, are well-suited to share these stories from others living in their communities and use them in discussion grounded in motivational interviewing. We will test a novel intervention to help persons with uncontrolled HTN initiate and sustain behaviors to control their HTN.

Family Health Center of Worcester and Lowell Community Health Center

Target Population:
Individuals diagnosed with hypertension
